Open vascular surgery is indicated when the vascular problem requires a solution from an ‘outside to inside’ approach and is traditionally performed in the operating room, under general anesthesia. This usually needs one or more incisions and remains the gold-standard for treatment and outcomes. While the results of the operation tend to be most durable, open operations have slightly more morbidity (pain, down-time, healing) and risk of other complications. Furthermore, patients who are elderly or those who have many other medical problems (co-morbidities) may not be fit enough for open surgery.


Endovascular surgery is minimally invasive and approaches the problem from inside the blood vessel, often through a needle stick. Once inside, a guide wire can be passed across the lesion and treatment can be delivered precisely and accurately. These include balloon angioplasty or stenting of segments with narrowings or blockages in arteries, and ablation therapy for malfunctioning veins in varicosed veins. Most procedures are well tolerated by patients as there are no big incisions to recover from, morbidity (pain, down-time) is low, and sicker or older patients can be treated safely where they would otherwise disqualify from open vascular surgery. Occasionally, a combination of open and endovascular (hybrid) surgery is planned for the patient as the most optimal approach to achieve the best results for patients.
